For Canadian patients who would like a formal estimate prior to their imaging appointment, please provide the following information:

- Physician’s order/requisition
- Email address
Please email the requested information and a copy of the physician’s order/requisition to:
Once we receive the requested information, our Radiology Department will review the physician’s order and provide a formal estimate for the imaging services requested.
Billing Information
Patients receiving imaging services at Unity Medical Center will receive two separate bills:
- Unity Medical Center
- Technical Fee (Imaging)
- Professional Fee (Interpretation by Radiologist)
*Full Payment is expected the day of the exam*
